Victoria was admitted from Riverside Community Hospital and then transferred to Kindred Rancho. When she first arrived, Victoria was very weak, unable to bare any weight on her own, speak, drink or eat. In the hospital, Victoria was seen by our interdisciplinary team which includes Nursing, Rehab (PT/OT/ST), Dietary, Respiratory Therapist, MD and Pharmacy.
During the initial rehab assessment, she presented as very weak throughout her body, required total assistance with bed mobility, was unable to transfer and walk or complete self-care tasks and could not speak or eat.
With rehab training and Victoria’s determination to get better, she began to improve functionally until she was able to move in bed without assistance, stand and move from one place to another with moderate assistance and walk with min-moderate assistance using a front-wheeled walker up to 250 feet. Victoria has also improved her self-care skills with minimal to no assistance. Lastly, she was able to eventually speak, drink and eat regular texture foods with thin liquids.
Victoria will be discharged to an Acute Rehabilitation Hospital so that she can further improve functionally and allow her to return to her previous routines. She and her daughter were very pleased with the improvement she attained at Kindred.
Care for Complex Conditions
Patients come to our hospitals with serious conditions such as respiratory failure, complex wounds, sepsis, stroke, congestive heart failure, or even a combination of these. Their diagnoses are often complicated by existing health problems like diabetes or COPD. Many of our patients require treatments such as mechanical ventilators, dialysis, or IV therapy.
