Referring a medically complex patient to one of our Kindred Hospitals after they have been stabilized can help expedite recovery and reduce the risk of readmission. 

If you have a medically complex patient who may benefit from a referral to a Kindred Hospital, please use one of the below options to get started.

 

Patient Referral Options

  1. Call 866.KINDRED to start a patient assessment today
  2. Complete the contact form below and we will call you within 24-48 business hours
  3. Refer online via Allscripts Referral system or similar program (participating healthcare facilities only)
 
 

When to Refer a Patient

Because we offer ICU-level care, appropriate patients can be transferred to a Kindred Hospital while they are still in a critical condition. Referring physicians may have the option to follow their patent at our facility for continuity of care.

Timely referral to a Kindred facility, where patients will be treated by caregivers that specialize in complex conditions, decreases inpatient length of stay while improving outcomes.
